Hard copies of the application are available at the Vital Statistics window. Use this form to apply for:

Certification: A document "for informational purposes only" (no raised seal).

Certified Copy: Also known as the "Long version," a certified copy will contain the raised Seal of the Township of Willingboro and can be used for legal purposes. (This document is usually obtained to get a Passport application, Military benefits, Insurance claims, and Driver's licensees).

Instructions

To obtain a vital record, please remit your application, payment, and supporting documents in person at the Vital Statistics window in the Municipal Building.

For mail order requests, submit a brief letter indicating the person's birth name, date of birth, mother's name: including mother's maiden name, father's name, and a clear copy of your photo ID.

Fees & Payment

There is a $25 fee for a Certified Copy / Certification of Vital Record application. Personal checks, money orders, and cash are accepted.

Documentation

The required documents to obtain a Certified Copy / Certification of Vital Record application are:

• A valid driver's license, or
• State issued identification, or
• Two alternate forms of identification with name and address.
• The $25 fee for every record requested.
