Monday - Friday 9:00AM -4:30PM for walk-ins. (Please note on the 1st and 3rd Tuesdays, services are by appointment only.) For more information, contact the Registrar at 609-479-1221.
Hard copies of the application are available at the Vital Statistics window. Use this form to apply for:

Instructions
Death certificates are filed in the township where demise was officially pronounced.
To obtain a certificate, please remit your application, payment, and supporting documents in person at the Vital Statistics window in the Municipal Building.
The immediate family or Executor/Executrix must show identification in order to secure a certified raised sealed copy.
(Immediate family members includes the following ONLY: Mother, Father, Sister, Brother, and Children).
Fees & Payment
There is a $25 fee for the initial certificate and a fee of $10.00 for every copy requested thereafter.
Credit cards, personal checks, money orders, and cash are accepted.

Documentation
The required documents to obtain a Death Certificate(s) are:
- A valid driver's license or state issued identification and/or a valid passport.
- For more information regarding acceptable forms of identification, visit Department of Health | Vital Statistics | Acceptable forms of identification.
- Children of the deceased must provide original birth certificate (that includes parent's name) for proof of relationship.
- All required documents must be original.
- The fee is $25.00 and $10.00 for each additional Death Certificate.
